Is it possible to save karma

I think you have to rewind to the point where the first strike force mission becomes available. I restartet the whole campaign and collected the intel. I replayed each mission using rewind and didnt get the achievement so it looks like it has to be a fresh new playthrough. You said second chance dose not become available if I kill DeFalco so how am I supposed to kill him and play second chance? I rewound to "Karma" and completed it, killing DeFalco. It gave me a warning that I had 1 mission left to complete the Strike Force, but I couldn't select any missions other than the Panama one that follows "Karma.
